Don't forget your id!
Adults (18+): Valid driver's license, state ID, or passport required. A temporary paper ID is accepted only alongside your expired ID—expired ID alone won't work.
Minors (8–17): Both the minor and accompanying parent/guardian need valid ID.
- Parent/guardian: State ID, driver's license, or passport
-Minor: Birth certificate, state ID, driver's license, or passport
-School IDs not accepted
If last names or addresses don't match across IDs, we'll also need a birth certificate. Judge-signed court documents verifying guardianship work, too.
All documents must be originals—no copies or photos.
Late Arrivals + Documentation
Arriving more than 5 minutes late or not having the required documentation will be treated as a cancellation, and your deposit will be retained. If you’re running late, call us 815-782-2533 — if availability allows, we may be able to move you to a later time.
By clicking the box below, you are also confirming that the person who will be getting pierced is not currently taking immunosuppressants, chemotherapy medication, Accutane (Isotretinoin), anticoagulants (Warfarin or Rivaroxaban), or corticosteroids.
