A labret piercing sits just below the lower lip, centered above the chin. It is one of the cleaner, more architectural placements in facial piercing — structured enough to feel intentional, subtle enough to wear into almost any setting. But getting there requires more thought than the minimalist result suggests.

Labret placement is shaped by the anatomy of your lip and surrounding tissue. The curve of your lower lip, the thickness of your tissue, and the visibility of underlying blood vessels all affect where the piercing should sit and what jewelry will actually work on your face. Some placements that look standard on paper do not translate cleanly to a specific anatomy — and that is worth knowing before anything is marked.

The labret sits in a highly visible location, which means both the aesthetic and the anatomical have to be right. Blood vessel location, tissue depth, and lip curvature all affect where the piercing should sit and whether a given placement will heal cleanly. A few millimeters in the wrong direction can create long-term irritation or a result that never quite looks intentional.

We pierce labrets with 16g ASTM F136 implant-grade titanium. Initial jewelry is a flat back post with a threadless push-pin top, with post length typically falling around 7/16” (11mm) or 1/2” (13mm) depending on your tissue and anatomy.

Flat back posts are the standard for labret piercings because the flat internal disc sits comfortably against the inner lip tissue, reducing pressure and movement during healing. Threadless push-pin tops are secure, low-profile, and easy to change once the piercing is fully healed.

Initial sizing leaves room for swelling without excess length that would create unnecessary movement. We size based on your anatomy, not a default.

Hoops are an option once the piercing is fully healed — typically at 12 months or more.

We begin by examining your lower lip — tissue thickness, the curve of your lip line, and whether any blood vessels are visible in the area. Labret piercings can be anatomically specific, and this assessment shapes everything that follows.

Once placement is marked and approved together, the piercing is done with a needle. Some piercers work freehand; others use forceps depending on anatomy and what gives the most accurate result. A taper guides the jewelry into place. Calm, considered, and unhurried from start to finish.

What Makes a Labret Different

The labret is a flat surface piercing in the sense that it sits in soft tissue below the lip rather than passing through a defined cartilage structure. That means the jewelry type matters significantly — flat back posts are the standard because they sit flush against the inside of the lip, reducing irritation and movement during healing. The visible end sits outside; the flat disc sits inside, resting against the inner lip tissue.

This placement also means healing is affected by daily habits in ways other piercings are not. Eating, talking, and anything involving contact with the lower face all interact with the piercing during the healing period. That is not a reason to avoid it — it is a reason to place it carefully and give it the time it needs.

Healing Time: 3-4+ months

Labret piercings heal more quickly than cartilage but require consistent care throughout. The tissue is in near-constant motion — eating, talking, and daily expression all affect the healing environment. Even when things feel settled on the surface, the tissue underneath may still be working. We will walk you through realistic expectations so nothing feels surprising.

Downsizing

A piercing starts in the studio, but the healing process is each client's responsibility and can’t happen here. At Stacked, we require our clients to come back in for a six-week checkup. This way, we can help troubleshoot any concerns you may have, take a peek at the healing piercing, and give advice as needed.

Once swelling has resolved, initial jewelry that is too long creates unnecessary movement against the tissue. Downsizing at the right moment is one of the most important things you can do for a labret piercing during healing. We will guide the timing based on how your piercing is responding, not a fixed schedule.
