Nipple Piercing

A nipple piercing is one of the more personal piercings we do. It is also one of the more anatomy-specific ones — and that combination means the experience, the placement, and the outcome all depend on doing things carefully from the start. If your anatomy is not a good fit for a nipple piercing, we will be honest with you about that. 

Nipple anatomy varies more than most people realize. Size, shape, projection, and tissue density all affect where a piercing should sit and what jewelry will actually heal well on your body. Very small or inverted nipples change the approach. What works for one person may not work for another, and a studio that treats this as a standard procedure tends to get standard — or worse — results. If your anatomy is not a fit, we will be honest with you about that.

Why Get a Nipple Piercing at Stacked?

A nipple piercing that is placed incorrectly — wrong angle, wrong depth, wrong jewelry length — is more likely to migrate, cause prolonged irritation, or never fully settle. These are not rare outcomes when placement is rushed. They are predictable ones.

Very small nipples require adjusted placement and jewelry sizing to avoid surface-level rejection. Inverted nipples can often be pierced, but the process and expected result look different, and that conversation happens before anything is marked. Flat nipples sit somewhere in between — pierceable in most cases, but only with precise placement and the right initial jewelry.

We do not pierce around anatomy problems. We work with what is actually there.

Single vs. Pair

Both are common, and both are done at Stacked. A single nipple piercing is a complete look in its own right. A pair is a natural complement — balanced, intentional, and something many clients choose to do together rather than return for the second.

If you are considering a pair, doing both in the same appointment is generally easier than coming back for the second. Healing runs concurrently, aftercare is the same, and you only go through the pre-piercing process once. The appointment is a little longer, and the fee reflects that — but the experience is designed to be just as calm and considered as a single.

We pierce nipples with 14g ASTM F136 implant-grade titanium. Initial jewelry is a straight barbell with internally threaded tops, with sizing typically falling around 5/8″ (16mm) or 3/4″ (18mm) depending on your tissue and anatomy.

The straight barbell is the standard for initial nipple piercings because it sits naturally across the base of the nipple without creating uneven pressure on the tissue during healing. Initial sizing is intentionally generous to accommodate swelling, but not so long that the bar creates unnecessary movement.

Internally threaded or threadless posts are used because it protects the piercing channel during jewelry changes. With internally threaded jewelry, the threading is inside the post, making it a seamless transfer without snagging the tissue.

We begin by assessing your anatomy — nipple shape, tissue, and what placement and sizing will actually support long-term healing. Once we have a clear picture of what your anatomy can support, placement is marked and reviewed together before anything is pierced.

The piercing is done with a needle. Some piercers work freehand; others use forceps or a cork to stabilize the tissue depending on anatomy. Jewelry is then guided carefully into place. The whole appointment is calm, unhurried, and designed to feel as comfortable as possible. If you are nervous, that is expected — and we will talk you through every step.

Pain Level 5-8/10

Nipple piercings are among the more noticeable piercings in terms of sensation. The nipple has a high concentration of nerve endings, and most clients describe a sharp, brief intensity during the piercing itself followed by tenderness that settles over the following days. Pain varies significantly by anatomy, threshold, and the time of month for those with hormonal cycles. What almost everyone agrees on: it is over quickly.

Nipple Piercing Healing Timeline

Healing Time: 6–12+ months

Nipple piercings take time. The tissue is subject to constant contact from clothing, movement throughout the day, and friction during sleep — all of which affect how healing unfolds. Even when a nipple piercing looks and feels calm, the internal tissue may still be working through the process.

Healing is rarely linear here. There can be periods of calm followed by irritation in response to a snag, a tight bra, or pressure during sleep. Protective measures — breathable fabric, a well-fitted bra or binder, and avoiding direct water pressure — make a real difference in how smoothly the healing goes. We will walk you through all of it.

Downsizing

Downsizing a nipple piercing is assessed at your six-week check-up and done if the fit calls for it. Once initial swelling has resolved, a bar that is too long creates more movement than healing tissue needs, which can slow the process or cause irritation.

Because nipple anatomy varies, the right timing for any adjustment depends on how the piercing is sitting and how your body is responding. We do not follow a rigid schedule here — we follow the healing.
